The Supportive Housing Program SHP seeks to combat homelessness by providing intensive case management and client support services to assist families and individuals to become self-sufficient. Participants receive up to 24 months of assistance in resolving the crises in their lives, accessing appropriate community resources, and assistance with food and case management to achieve more stable independent lives. The Transitions Program seeks to combat homelessness and bridge the gap between temporary emergency shelter and permanent housing. Assists with the transition of homeless individuals and families from emergency shelter to housing in residential neighborhoods with the provision of tenant-based rental assistance, deposit assistance, intensive case management, and supportive services up to twenty-four months. Participants receive assistance in resolving the crises in their lives that prevent self-sufficiency, assistance in gaining access to needed community services designed to help clients achieve more stable independent lives, and assistance in transitioning into permanent housing. Eligibility: Homeless individuals or families who meet HUD income and program guidelines Fees: None Intake Procedure: Meet with case manager and complete application pack. Hours: Monday - Friday 8:00 am-5:00 pm
